Health Exchange of Arizona provides assistance to its clients in the comfort of their own home. We are an alternative to nursing homes or assisted living facilities. We have experienced knowledgeable and personable caregivers available throughout the Valley. Whether you are interested in assistance for yourself, a spouse or a family member you can depend on Health Exchange of Arizona’s high standards. Each caregiver must meet our qualifications, which includes a background check and drug screening. The caregivers are supervised directly by me and have support with issues or concerns on a 24 hour, 7day a week basis.
Once Health Exchange of Arizona’s short intake form is completed, I will contact you so we can then schedule an “Assessment Evaluation” visit in your home at your convenience, at no charge to you!
During this visit your needs will be discussed and together we will complete your individualized care plan. After receiving the results of my visit Health Exchange of Arizona’s experienced Staffing Coordinator will seek out the appropriate caregiver; they will then call you the (client) with the name of your caregiver and will confirm the hours you are requesting. Usually after a short period of time and with good communication a working relationship and routine will develop between you the client and your caregiver. The care plan is reviewed regularly and I am always available for both client and caregiver concerns.
Health Exchange of Arizona’s contract is simple and available on this website for your viewing. Care is provided to you, as you need it, our contract does not bind you to any commitment of hours. Financially, to begin services you have two options:
1. Health Exchange of Arizona requires a deposit check in the amount equal to one-week of service; this check will be deposited in a holding account and returned to you when services are terminated. Invoices for services rendered will be mailed weekly.
2. Use of your Visa or Master Card, will avoid the need for a deposit. Your card will be billed weekly and invoices will be mailed out to you by Health Exchange of Arizona.
